Every family an get Rs.5.00 Lakhs free medical treatment in any govt. hospitals. So in this Article i will show you the process of making a Ayushman Bharat Card or 5 Lakh wala Health Card. So lets Start:-

What is the Benefit of Ayushman Bharat Yojana (Health Card)

Every family can get this benefits if they are eligible for this scheme. If found they are eligible then Rs.5.00 to be given as medical treatment. This amount is not giving in cash in hand, the eligible family can get this benefit only use on medical treatment of any family members in their family.

How to Apply

I want to show you the process, how to check are you eligible for this scheme, then i will tell you the process of Apply of Ayushman Bharat Card in 2021.

How to Check you are eligible for this scheme or not ?

Here you can see a Question Mark (?) symbol is available in the top of the page. Just click here.

Now enter the Mobile No. and enter the captcha and click on Generate OTP Button. A OTP has been sent to you entered Mobile No. just enter the OTP and verify it.

Now enter the State Name and enter Beneficiary Name, city name and village and click on Search Button.

Then results will be show in the right side. If you are eligible then your name will be appear in the right side of the screen. Click on Family Details Tab.

Next screen will appear. Now click on Get Details on SMS Tab.

After enter the Mobile No. a SMS will be receive in your Mobile. Then you have to go to the nearest CSC Center for complete the process. Just go the CSC Center and show the SMS, then they will complete your Ayushman Bharat Card process.

This eligible list are based on Samajik Arthik Jati Jan Ganana 2011. But when you can search your name in this website but if no any results is appear, i mean if you are not eligible then how can you apply for Ayushman Bharat Card in 2021.

You have to visit CDMO Office, Every district has a CDMO Office, Chief District Medical Office. Go to the office and ask them for this scheme. All the details are available in the CDMO Office.
